PROMOTION MECHANICS:

1. This promotion is open to all Bonifacio High Street customers.

3. For a minimum single receipt purchase of P3,000, whether paid in cash or credit card, customers are entitled to avail of ONE (1) AXA Personal Accident Insurance card with coverage worth P50,000.

4. The purchase must be from any one of the participating Bonifacio High Street, One Parkade, Two Parkade & One Bonifacio Highstreet Mall establishment.

5. Transactions involving the following businesses are NOT qualified for the promo: casino/gambling, money transfers, quasi-cash financial institution, political organizations and bail and bond payment.

REDEMPTION MECHANICS:

1. The Free AXA Personal Accident Insurance coverage worth P50,000 card may be redeemed at the AXA booth at Bonifacio Highstreet B5 (near 9th Ave corner Bench) daily from 11:00 AM to 12:00 MN only.

2. Customers must present the following documents to qualify for redemption:

a. Official Receipts/ Charge Slip of the purchased items, worth P3,000 and up; and

b. Valid IDs (government IDs, driver’s license and/or passport)

3. AXA personnel shall collect the following information upon consent of the customer: Name, Contact Number and Email for record purposes and shall stamp the validated charge slip/ Official Receipt.

4. The AXA personnel shall release ONE (1) AXA Personal Accident Insurance coverage card.

5. If the customer fails to avail of the AXA Personal Accident Insurance coverage card within the redemption period, the Personal Accident card will be forfeited in favor of AXA Philippines with prior DTI approval.

6. The Free AXA Personal Accident Insurance coverage card is not convertible to cash. 

7. Last day of redemption will be on December 31, 2018.

8. In case of dispute, the decision of AXA Philippines with the concurrence of DTI shall be considered final,

per DTI-FTEB Permit No; 15056 series of 2018.

Unfortunately, the critical part of the validation process lie in the details of the receipt (transaction amount) therefore it must be in good condition. The AXA Personnel cannot validate / stamp an unreadable receipt therefore, they cannot release the FREE AXA Personal Accident Insurance coverage card.

Can I choose the design of the Personal Accident coverage card?
