4. Can I just go to the AXA website and take the Health Risk Score?

• You must take the Health Risk Score by going to http://bit.ly/2wtADQY or by clicking any of AXA Philippines’ social media post or ads communicating the promo. If you just want to know your health risk score, you can still take it via the AXA website but this won’t be subjected to a raffle entry.

5. After taking the AXA Health Risk Score, can I just talk to my AXA financial advisor and get additional raffle entries?

• You must wait for the call of the AXA financial advisor assigned to this promotion to earn raffle entries.

6. What are the prizes for this promo and when are the raffle draw dates?

• Minor Raffle Draw:

i.       A total of 1,000 winners will be chosen to win a three-game choice NBA League Pass code or a 30-day NBA League Pass code.

ii.      Winners of the minor draw are still part of the Grand Raffle Draw.

• Grand Raffle Draw:

i.      Four (4) winners will be chosen to win a trip to the USA to watch an NBA game live

ii.      The package is inclusive of the following:

• One (1) ticket for an NBA game

7. How will I know if I won?

• Validation Process:

i.      Winners will be notified within two (2) business days from the draw through e-mail, SMS, and/or phone call by an authorized Company representative. Announcements will also be made via the Company’s Facebook and Twitter accounts.

ii.      Grand raffle draw winners need to confirm acceptance of the prize within two (2) business days upon successful notification by AXA Philippines.

iii.      In case the e-mail bounces, the SMS is undeliverable, or the phone/mobile number is uncontactable, AXA Philippines will attempt to contact the winner (for grand raffle draw winners) or deliver the prize again within 48 hours. Should the attempt fail, AXA Philippines reserves the right to draw another winner or to forfeit the prize.

iv.      Upon acceptance of the prize, grand raffle draw winners need to submit the requirements within 10 business days or the prize may be forfeited.

v.      AXA Philippines reserves the right to conduct further validation and verification before awarding the prize.

8. I won the minor draw – how can I claim my prize?

• Winners will receive their NBA League Pass codes with instructions on how to activate via e-mail and/or SMS through the contact information they inputted in the quiz.

• Successful delivery of the e-mail and/or SMS constitutes acceptance of the prize.

• In case the e-mail bounces or the SMS is undeliverable, AXA Philippines will attempt to resend the NBA League Pass codes once. Should it fail for the second time, AXA Philippines reserves the draw another winner or to forfeit the prize.

9. I won the grand raffle draw – how can I claim my prize?

• Winners shall be the persons stated in the winning raffle entries.

i.      Winners must present the following requirements when claiming their prize:

• Two (2) valid, government-issued IDs or documents with winner’s full name, signature, picture, and home address (i.e. SSS, Driver’s License, PRC Card)
• The personal information should match the details submitted in the winning entry.
• Printed copy of the official e-mail sent by AXA Philippines
• Valid Philippine Passport (should be valid for use in the travel period specified by AXA Philippines)
• Valid USA Visa (should be valid for use in the travel period specified by AXA Philippines)
• Tax Identification Number (if applicable)

ii.      Winners should be considered healthy and fit to travel (acceptance of the prize constitutes that the winner meets this requirement)

10. If I win, when will the travel dates be?

Employees of AXA Philippines and Charter Ping An Insurance Corporation (“Sponsor"), NBA Properties, Inc., the National Basketball Association ("NBA") and its member teams (collectively, the "NBA Entities"), and their respective parent companies, subsidiaries, affiliates, dealerships, governors, related entities, partners, partnerships, principals, licensees, sponsors, and advertising/promotion agencies and all of their respective officers, directors, employees, agents, representatives, successors and assigns and their immediate families (parents, spouses, children and siblings and their respective spouses, regardless of where they reside), and household members, whether or not related, are not eligible to enter or win. 15.   Eligibility will be determined in accordance with these rules by the Sponsor, whose decisions are final in all matters relating to the Contest. Odds of winning the prize depend on the number of participating eligible entrants. In the event of non-compliance within any of the rules of the Promotion, the prize will be forfeited as against the relevant entrant.  In the event that no eligible entries are submitted, AXA PHILIPPINES will not give away the prize. The Sponsor reserves the right to substitute a prize for one of equal or greater value if the designated prize or any portion thereof should become unavailable for any reason.

16.   Definition of NBA Entities: NBA Properties, Inc., the National Basketball Association (the “NBA”), and the NBA member teams (collectively, the “NBA Entities”)

17.   Limitation of Liability: By participating in this Promotion, a Participant agrees and undertakes to release from any liability or hold free and harmless AXA Philippines and the NBA Entities and each of their respective parent companies, subsidiaries, affiliates, officers, directors, employees, governors, owners, distributors, retailers, agents, assignees, advertising/promotion agencies, representatives, and agents (“Released Parties”) from any claim, action, liability, loss, injury or damage, including, without limitation, personal injury or death to winner or any third party or damage to personal or real property due in whole or in part, directly or indirectly, by any reason, including the acceptance, possession, use or misuse of the prize and/or participation in this promotion.
